go to Seibu Shinjuku station(西武新宿駅) for Marunouchi line(Metro)(丸の内線) to Tokyo Station(東京駅)
switch to Keiyo Line(JR)(京葉線) to Maihama Station(舞濱駅). (take the rapid to skip a few stations)
becareful not to accidentally choose Keio Line(京王線)
from Maihama Station(舞濱駅), there's a monorail that'll take you directly to Disneyland.
