Usually HJ is $20 but you may get it included or just $10.
BS is from $40 to $80 on top of HJ. But, someone will offer it for $20. It all depends or you can negotiate.

C-Hing, if you go more to MP and SP place, you'll have the answer yourself. There are a lot of competition and also requirements from customers. Based on the condition of providers, there's lot of variety.
I can say FS is not always provided by MP. Try going to a good skill MP, you'll know why she provides extra or FS or not. However, you may not take FS from them. For those good SP, don't expect a good massage skill. Each of them has a selling point and you go for what you want.
